Video (DVD) Bookings

Now is the time to start thinking about those videos that you may need for your classes next semester. We have a Video Booking Request form that is to be used to book any videos for classroom presentation in advance of the class. We do need a minimum of 24 hour advance notice.

This form may be used to book any number of videos and in fact, if possible, it is best to book all of your videos for the entire semester. You would just have to indicate the different dates clearly, so they can be booked for the appropriate dates.

Since the form is only for advanced booking, the form is not required.

However, if we have advance notice of the need for a video, we try as much as possible to make sure that the video is available when requested. Usually, we will not allow videos to be checked out by other faculty during a booking date. Of course, one of our barriers to this service is when faculty do not return the videos when they are due which then could carry over into a booking date. However, booking in advance is more of a guarantee that the video will be available on the day requested.

Our Video Booking Request form is available online on our webpage at http://docuserv.auraria.edu/eres. Just click on the link “Online forms” and look for the link “Video Booking Request Form”. The form can be printed out from your office printer and even filled out online and printed off. You can drop it off at our service desk, send to us by mail or even fax it to us at 303-352-3886.

From the Online forms link, you may also access a permission letter which will allow others authorized by the faculty member to pickup a video for a faculty member. The form requires the faculty Id number and signature. If the person picking up the video presents this letter to our desk personnel, there is no need for the faculty ID card.

However, the faculty member, by signing this permission letter, accepts responsibilities for the video as it will be checked out in the faculty name and the faculty member is responsible for any fines or replacement charges due to overdue or lost items. It is best to have a new letter for each item requested but we will accept one form from the faculty member indicating that a specific person is authorized to pickup items for the faculty member for the entire semester, if this is noted on the form. We do keep all original forms on file for the semester. The person picking up the video should be prepared to present a current Id or a course schedule and driver's license.
